What is Vial of Life?
It is information that you store in a plastic bag on the front door of your refrigerator, so if an emergency happens, the people who come to help you can quickly learn what they need to know to provide the best help possible.
What goes in the plastic baggie on the front of your refrigerator door?
A completed Vial of life form
Providing a picture of yourself would be helpful.
A Photostat of your last EKG will greatly assist emergency personnel. (Your doctor should gladly provide you a copy)
Place your Living Will or equivalent in your baggie, if you have one.
Place any Do Not Resuscitate (DNR) documentation in the baggie, if you so choose.
Place any other documentation you feel important in the baggie.
Place a Vial of Life sticker on the bag.
Tape the bag to the front of your refrigerator.
Place a Vial of Life sticker on your front door.
Take it with you when you go to the doctor
During normal office visits to any of your doctors, the Vial of Life will tell them what medications you are taking, and the ailments that you have. These are used telephones that are only capable of calling 911, they are not able to receive any calls. They are intended for emergency use only and are not allowed to be sold or re-activated by a Cellular Service Provider.
To obtain a phone, you must be a senior citizen and will need to attend an informational training session. Location & time of these sessions will be announced later.
Phones are limited and will be distributed first to Seniors who have no other telephone service and then on a first come basis.
